Observation Chart
A clinical tracking tool for dog trainers and behaviourists. Set up once at consultation — the owner logs daily scores at home, and a full report is generated automatically.
How This Tool Works — Read Before Setup
1
Practitioner setup (now): Enter the dog's name, then define 3 behaviours you want to increase (e.g. calm settle, loose lead walking) and 3 you want to decrease (e.g. jumping up, barking at strangers). These are set once.
2
Daily owner logging: Each day, the owner opens this page on their device and scores each behaviour from 0–5. This takes under 2 minutes.
3
Score meaning:
0 Not shown at all 1 Rarely 2 Sometimes 3 Often 4 Frequently 5 Constantly / maximum

For desired behaviours, scores should trend upward (5 = excellent). For unwanted behaviours, scores should trend downward (0 = excellent).
4
After 7 days: A full PDF report is automatically generated with progress charts, trend analysis and a clinical summary — ready to email to the professional.
5
Data storage: All data is saved only on this device. Nothing is sent anywhere automatically. The device should not be cleared or reset during the tracking period.
